What is SPRAVATO (esketamine)?
SPRAVATO is a prescription nasal spray containing esketamine, approved by the FDA in 2019 for adults with treatment-resistant depression. You spray it yourself in a certified clinic while a clinician observes, then stay for at least two hours of monitoring before someone drives you home.
What is treatment-resistant depression?
Treatment-resistant depression is major depressive disorder that hasn't improved enough after at least two different antidepressants, each taken at an adequate dose for an adequate length of time. It is common, and it is a reason to change the approach rather than repeat it.

Treatment-resistant depression, in plain terms

Most antidepressants and SPRAVATO start in different places
Esketamine is a non-competitive NMDA receptor antagonist. Exactly how it relieves depression isn't fully understood, which is part of why every dose is given under supervision.
Two FDA-approved uses, both in adults
- Treatment-resistant depressionOn its own or together with an oral antidepressant. The on-its-own option was added in January 2025.
- Major depression with acute suicidal thoughts or behaviorTogether with an oral antidepressant. It hasn't been shown to prevent suicide, and it doesn't replace a hospital stay when one is needed.

Your prescriber chooses a dose of 56 mg or 84 mg. Each single-use device holds 28 mg, one spray in each nostril; when a dose needs more than one device, you rest five minutes between them. From week 9 the goal is the least frequent schedule that keeps you well.

Finish eating at least 2 hours before and stop drinking 30 minutes before, it helps with nausea. If you use a nasal steroid or decongestant, take it at least an hour ahead. Confirm your ride.

Boxed warning, summarized
SPRAVATO carries a boxed warning for sedation, dissociation, respiratory depression, and abuse and misuse. Antidepressants also raise the risk of suicidal thoughts and behaviors in children and young adults, and SPRAVATO isn't approved for anyone under 18. Tell us right away if your mood or thoughts get worse.
SPRAVATO isn't used if you have
- An aneurysm or aneurysmal disease of the blood vessels (including the aorta, brain or limbs), or an arteriovenous malformation
- A history of bleeding in the brain (intracerebral hemorrhage)
- An allergy to esketamine, ketamine or any ingredient in the spray

SPRAVATO is dispensed and taken only inside our office at 2100 N Hwy 360, Ste 405B, Grand Prairie, TX 75050. Every dose is self-administered while a clinician watches, every patient is monitored for at least two hours afterwards, and SPRAVATO is never sent home with you.
Adults with major depressive disorder whose depression has not improved enough after at least two antidepressants, each taken at an adequate dose for an adequate length of time. SPRAVATO is also approved, together with an oral antidepressant, for adults with major depression who have acute suicidal thoughts or behavior. It is not approved for anyone under 18, and it is not used in people with certain blood-vessel conditions, such as an aneurysm or arteriovenous malformation, or a history of bleeding in the brain. A psychiatric evaluation confirms whether it fits your history.
Not necessarily. Since January 2025 the FDA label allows SPRAVATO for treatment-resistant depression either on its own or together with an oral antidepressant. For depressive symptoms with acute suicidal thoughts or behavior, it is used together with an oral antidepressant. Your prescriber recommends the approach that fits your history. Please do not stop any medication on your own.
Plan on about two and a half hours. You check in and have your blood pressure taken, give yourself the dose while a clinician watches, then rest for at least two hours while we monitor you. Blood pressure is checked again about 40 minutes after the dose, and you leave once your clinician confirms you are stable.
For treatment-resistant depression the labeled schedule is twice a week in weeks 1 to 4, once a week in weeks 5 to 8, and then once a week or every other week, using the least frequent schedule that keeps you well. At the end of the first four weeks your clinician reviews whether continuing makes sense. How long treatment lasts after that is individual.
No. SPRAVATO can cause sleepiness, dizziness and a detached feeling, so you should not drive or operate machinery until the next day after a restful sleep. Arrange for a trusted friend or family member to take you home after every session.
Medicare and many commercial plans can cover SPRAVATO for treatment-resistant depression, usually after a prior authorization that documents the antidepressants you have already tried. The most common effects are feeling detached or outside yourself (dissociation), dizziness, nausea, sleepiness, a spinning sensation, numbness, anxiety, low energy, a rise in blood pressure, vomiting and feeling drunk. They tend to appear soon after the dose and ease on the day of treatment, which is why you rest and are monitored in the office. Tell your clinician how each session felt so the plan can be adjusted.
It varies. In clinical studies some adults saw their depressive symptoms improve within days, others needed the full four-week induction phase, and not everyone responds. Your clinician formally reviews your progress at the end of the first four weeks and decides with you whether to continue.
They are related but not the same. SPRAVATO contains esketamine, one mirror-image form of the ketamine molecule, made as a nasal spray and approved by the FDA for depression. Ketamine infusions for depression are an off-label use. SPRAVATO follows a labeled dosing schedule, is given only in REMS-certified settings, and can be billed to insurance.
Often, yes. Your first psychiatric evaluation and follow-up medication visits can usually happen by secure video from anywhere in Texas. The SPRAVATO sessions themselves always happen in person at our Grand Prairie office, because every dose has to be taken and monitored on site.
